Insurance & Payments

Uplift AT offers multiple ways for you to pay for services including in-network insurance billing, out-of-network or private pay with superbills, DDA funding, and outside grants. Give us a call to go over your specific situation!

In-network Insurance

Uplift AT is in-network with Premera Blue Cross and Lifewise. That means we are contracted to provide services at a specific rate through those companies. We have a published Fee Schedule that you may reference to estimate a maximum cost. However, the Fee Schedule represents our out-of-network private pay rates and in-network rates may be lower. When getting started, we’ll check your insurance to provide a more accurate estimate of costs.

We are in the process of becoming in-network with additional insurance companies currently. We will update this section once credentialing is approved.

Out-Of-Network Insurance

Being out-of-network means we are not contracted with your specific insurance company. You may still see us, however, with two options:

  1. Pay for services out of pocket without attempting reimbursement from your insurance company OR
  2. Pay for services out of pocket and then submit a superbill to your insurance company for reimbursement. We will provide you with a superbill and you are responsible for checking out-of-network reimbursement eligibility with your plan.

If you have a Medicaid plan that we are not in-network with, please contact us to discuss how we may be able to serve you through DDA waivers.

DDA Waivers

Individuals who are enrolled in the Developmental Disabilities Agency program may be eligible to a service waiver. These waivers cover a variety of services including Assistive Technology and Staff & Family Training. Uplift AT is contracted with DDA/DSHS to provide these services.
